Go to ID Card Office Online Appointment Scheduler
ID Card Office Online (osd.mil)

Select "Continue" under ID Card Office Locator & Appointments

Search for Site by Address

Enter Location -- Niagara Falls, NY, 14304, USA

Select unit preferred and click on "Schedule Appointment"

If date has available times, the block will be green with available times left

Click on the date and time you want

Fill out your information

Submit

Required documentation:

Two forms of ID are required for sponsors and dependents 18 years of age and older when enrolling a member or dependent in DEERS or when issuing an ID card. One form must be a valid photo ID. Other forms of ID include:

Military ID

Drivers license 

Foreign drivers license

Social Security card

Birth certificate

Another form of federal, state or local government issued photo ID

US or foreign passport
